The Team: What we do

The Community Rehabilitation and Enablement team (CREST) based in Christchurch is a dynamic, fast-paced, community-based team providing short-term rehabilitation and enablement interventions to reduce the number of admissions of older persons to hospital and residential care. The CREST team also supports patients to leave hospital and continue their recovery at home.

The Role: Where you fit in

We are excited to have recently created a new designated position, CREST Allied Health Clinical Co-ordinator which will assist to continue to develop the responsiveness and effectiveness of the CREST allied health service.

We are looking for an experienced Occupational Therapist or Physiotherapist who is passionate about supporting people to live well at home. The successful candidate is looking for a new challenge in service leadership to grow their clinical and leadership skills by leading this team, supported by the Community Clinical Manager.

In this role you will retain a clinical caseload as well as co-ordinating the allied health team to assess people in a timely way. You will establish rapid management plans focused on supporting rehabilitation and enablement through self-management, optimising of patient goals and independence. This role will also involve the day to day triaging of referrals to the allied health team to ensure that each client receives timely and appropriate support.

You will be expected to act as an ambassador for our service and be calm and confident when dealing with challenging situations and collaborate across an interdisciplinary team; communicating with various parties to ensure that we deliver a seamless service.

Applicants need to demonstrate motivation and commitment to providing high quality patient centred care and be able to demonstrate ability to be innovative and manage change. Experience of working with frail elderly patients is essential and experience of working in the mental health sector would be an advantage.

This is a designated position on the Allied Health and Technical salary scale.

To be considered for this role you will need to hold a current Annual Practising Certificate in an allied health profession (Occupational Therapist or Physiotherapist) that is recognised by the respective New Zealand Regulatory Authority and professional body.
