Health Improvement Practitioner – Auckland Region

Do you want to make a difference to health outcomes of whānau in your community?

Working in the general practice team to support whānau to achieve their health goals

We are looking for people with a strong cultural awareness and talk therapy background to support whānau to achieve hauora oranga.

 

This is a diverse and exciting new position that offers an impactful range of wellness support from within the General Practice teams. You will be working alongside whānau, hapu, and hapori to help them gain the knowledge, skills and confidence to positively engage in their hauora journey. The role utilises Focused Acceptance Commitment Therapy (FACT) to provide brief assessment, treatment planning, and delivery of a range of health and social interventions.

 

Working as a Health Improvement Practitioner offers a busy but incredibly rewarding environment, whereby you will get to see the difference that you can have on whānau.

 

You will:

 

Be a registered health professional, such as a social worker, occupational therapist, psychologist, or DAPAANZ counsellor

Have experience delivering talking therapies such as ACT or CBT in a culturally safe manner

Have a sound understanding of Māori and Pasifika models of health (te whare tapa wha, fonofale)

Be keen to proactively reach whānau in your community
