Location: Hutt Hospital – Community Older Persons, Rehabilitation and Allied Health team
Contract: Fixed term to June 2021
Hours:. 5 hours per day four days of the week (0.5 FTE)
ABOUT THE ROLE
An exciting opportunity for an Occupational Therapist to support innovation in development of our community services.
This is a new position established to work with our Medical/Acute Early Supported discharge (ESD) team and community referred clients requiring brief interventions. The role will be fixed term to June 2021 as we are continuing to develop the service and new opportunities may arise.
Our Med/Acute ESD includes two Registered Nurses and an HCA providing brief follow up for people presenting to the Emergency Department or discharged from our medical wards. They would love to have an Occupational Therapist working alongside them!
The focus of the service is to ensure our people are able to stabilise successfully following an acute episode and to offer restorative interventions to optimise well-being for both the client and care giving whanau.
You will need to bring experience of working in both acute and community settings, along with skills in occupational assessment of people in their own homes, many of whom are older adults with multiple health conditions.
Your recommendations will guide intervention planning by the ESD team, along with other providers who may have a role in supporting your clients. Efficient time management and excellent communication skills will be critical, along with a sense of humour and a flexible approach to your day!
The ESD Service is part of the broader Te Herenga Tangata Community team staffed by a range of Allied Health disciplines, Nurses and our very valuable Assistants. We also work closely with our medical and inpatient DHB colleagues and are extending our relationships with Primary Health partners.
You will find professional support from your Occupational Therapy colleagues within the team and the Professional Leader for Occupational Therapy.
